Department of Transportation (DOT) Physicals

We offer no-hassle, A.S.A.P. DOT exams for commercial driver’s license (CDL) holders and non-CDL drivers hauling enough weight to require a physical. Dr. Werth is a Federal Motor Carrier Safety Administration (FMCSA)-certified medical examiner with your best interest in mind and can provide tools and resources to build healthier habits to protect your livelihood well into the future.

What does the physical consist of?

- Health review

- Height and weight

- Blood pressure and pulse

- Hearing and vision

- Urinalysis (protein, blood, sugar, and water-electrolyte balance only)

- Systems exam (strength, sensory, heart, lungs, hernia, etc.)

- If needed, refer for additional testing

Following a completed evaluation, you will receive a determination for your commercial driver's medical certification (Form MCSA-5876) and instructions for self-certification.

How much does a DOT physical cost?

$85.00. No hidden fees or taxes. If a brief follow-up is required for determination, the visit is included in the first service charge.
