- Urinalysis

- If needed, refer for additional testing
Following a completed evaluation, you will receive a determination for your commercial driver's medical certification (Form MCSA-5876) and instructions for self-certification.

$85.00. No hidden fees or taxes. If a brief follow-up is required for determination, the visit is included in the first global service charge.
